Key Responsibilities- Plan and Manage Patient Care: Develop and implement individualized care plans to meet the unique needs of each patient.
- Conduct Patient Assessments: Gather and record patient medical history, vital signs, and physical condition to inform care decisions.
- Provide Routine Care: Administer medications, perform vital sign checks, and provide basic care to patients as needed.
- Monitor and Report: Continuously monitor patient vital signs and report any abnormalities to the case manager and physician.
- Document and Communicate: Accurately document patient care and communicate with family members, case managers, and physicians to ensure seamless care coordination.
- Order Supplies and Medications: Ensure adequate stock of medications and supplies to meet patient needs.
- Maintain Infection Control: Implement and enforce infection control policies to prevent the spread of communicable diseases.
- Education and Licensure: Completed post-secondary coursework and proof of current LPN licensure in good standing.
- Knowledge and Skills: In-depth understanding of patient care policies and regulations, excellent interpersonal skills, and ability to work independently.
- Travel Requirements: Moderate amount of travel between facilities may be required.
