Vermont Case Manager Part-time Educator Virtual Contracted Position

We are seeking a passionate and experienced Case Manager to join our team at Soliant. As a Case Manager, you will work remotely with a K-12 caseload, providing essential support and guidance to students and their families.
Key Responsibilities- Manage a caseload of K-12 students, providing individualized support and case management services.
- Collaborate with educators, parents, and other professionals to develop and implement effective intervention strategies.
- Conduct assessments and develop personalized education plans (IEPs) for students.
- Monitor student progress and adjust strategies as needed to ensure success.
- Maintain accurate and up-to-date documentation of services provided and student progress.
- Participate in virtual meetings and trainings to stay current with best practices and policies.
- Master's degree in Social Work, Education, or related field.
- Experience in case management or working with K-12 students.
- Strong communication and interpersonal skills.
- Ability to work independently and manage a remote caseload effectively.
- Familiarity with virtual platforms and technology used in remote education is a plus.
- Current licensure or certification relevant to case management in Vermont.
