Job Title: MPI Community Behavioral Health Specialist

Metropolitan Family Services is seeking a highly skilled and compassionate Community Behavioral Health Specialist to join our Communities Partnering 4 Peace (CP4P) Team. As a key member of our team, you will play a vital role in providing comprehensive, in-depth assessments and case management services to individuals and families in need.

Job Summary:

The Community Behavioral Health Specialist will work closely with community-based organizations, schools, and other stakeholders to identify and address the root causes of violence and gang activity. You will provide a full range of case management, counseling, advocacy, educational, and related services to CP4P participants, including:

  • Comprehensive assessments and intake
  • Case management and counseling
  • Advocacy and support
  • Education and training
  • Referral network development
Responsibilities:

The Community Behavioral Health Specialist will be responsible for:

  • Providing comprehensive, in-depth assessments for a variety of clinical services
  • Performing psycho-social assessments and intake as assigned
  • Providing a full range of case management, counseling, advocacy, educational, and related services to CP4P participants
  • Carrying a caseload of clients
  • Providing support for MPA
  • Providing trainings and facilitation
  • Identifying and utilizing agency and community resources
  • Assessing community conditions affecting clients to identify resource gaps and problems
  • Joining or developing groups and coalitions to address community needs
  • Recommending financial assistance for clients
  • Providing group services as indicated by program need
  • Responsible for meeting contractual requirements such as 24-hour crisis coverage
  • Representing and interpreting programs of agency in community
  • Support and attend 'Light in the Night' events
  • May serve on regional or agency-wide committees
  • May provide clinical guidance and shadowing opportunities for students
  • Attend regular meetings: e.g. team, case consultations, individual supervision
  • Provide ongoing support, trainings, facilitation and curriculum development
  • Establishes referral network/linkages with outside agencies for participants and their families
  • Takes a proactive role in the development of social work services and programming that supports trauma-informed care
  • Meets agency expectation for quantity of direct service
  • Complies with agency record-keeping expectations
  • Works effectively with diverse staff and service population
  • Flexible to travel to different neighborhoods of the city
Qualifications:

To be considered for this role, you must have:

  • A Master's Degree in Social Work from an accredited college or university
  • Two years related post-Masters experience at an intermediate skill level
  • A Licensed Clinical Social Worker (LCSW) or related license
  • Requires mobility to work with a community-based caseload
  • Ability to maintain assigned community and home-based services
  • Ability to work effectively with diverse individuals and groups
  • Bilingual Spanish preferred
Physical Demands:

The Community Behavioral Health Specialist will be required to:

  • Sit and stand for sustained periods of time
  • Frequently talk, hear, use hands and fingers to feel, handle and operate objects or controls
  • Occasionally stand, bend, stretch, stoop, kneel and crouch
  • Have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing and viewing a computer terminal
  • Lift and/or move up to 25 pounds
Salary:

Compensation is commensurate with years of related experience, positions requirements, and candidate qualifications.
