The RN or LPN Case Manager is a key member of our healthcare team at The Highlands, responsible for coordinating patient care and services. This role requires a collaborative approach to ensure high-quality care, working closely with patients, families, care teams, payers, and external entities.
Key Responsibilities- Coordinate patient care and services, promoting quality care through a collaborative process.
- Work with patients and families to understand their needs and develop personalized care plans.
- Collaborate with care teams, payers, and external entities to ensure seamless transitions and optimal care outcomes.
- Develop and implement care plans that meet the unique needs of each patient.
- Monitor and evaluate patient progress, making adjustments to care plans as needed.
- Communicate effectively with patients, families, and care teams to ensure clear understanding of care plans and progress.
- Stay up-to-date with industry developments and best practices in case management and care coordination.
- Nursing diploma (associate's or bachelor's degree in nursing)
- Currently licensed/registered in applicable State. Must maintain an active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing throughout employment.
- One (1) year of clinical experience in post acute care setting preferred
- Prior case management, utilization review, and discharge planning experience preferred
- Certified Case Manager (CCM) or Board Certification in Nursing Case Management (RN BC) preferred
- Excellent communication and interpersonal skills
- Strong organizational and time management skills
- Ability to work independently and as part of a team
- Knowledge of industry regulations and standards
- Ability to adapt to changing situations and priorities
