Job Summary

We are seeking a skilled Community Health Worker to join our team at LifeBridge Health. As a key member of our healthcare team, you will provide direct services to individuals and families, extending psychosocial support services to promote health and well-being.

Key Responsibilities
  • Conduct assessments to identify psychosocial barriers to health and develop client-directed service plans
  • Provide education and support on health, medical compliance, budgeting, safety, and community resources
  • Collaborate with healthcare providers to ensure comprehensive care
Requirements

To be successful in this role, you will need:

  • A formal working knowledge equivalent to an Associate's degree (2 years college)
  • 1-3 years of experience in a related field
  • A driver's license and personal transportation (preferred)
