Senior Care Management
As a Nurse Care Manager for Senior Care Option at US Tech Solutions, you will be responsible for managing all aspects of senior care for a panel of frail elderly under administrative direction of Clinical Manager.
Key Responsibilities:
- Interface with physicians, members, caregivers, and healthcare teams to identify risk factors, conduct assessments, and develop care plans.
- Timely completion of geriatric assessments and development of individualized care plans.
- Completion of Minimum Data Set-Home Care (MDS-HC) and facilitation of access to community resources.
- Participation in primary care team meetings and proactive management of transitions of care.
- Direct caregiver support, serving as a member advocate, and providing education and coaching.
Requirements:
- Bachelor of Science in Nursing (BSN) and Registered Nurse with current Massachusetts license.
- Minimum 5 years clinical nursing experience and experience in case management and geriatric/chronic illness populations.
- Proficient in computer use and health information technology.
This role requires the ability to work cooperatively as a team member, demonstrate initiative, prioritize work, and maintain confidential data. Travel to members' homes and other sites is required.
