Job Summary

We are seeking an experienced Clinical Care Coordinator to join our team at Mass General Brigham. In this role, you will be responsible for coordinating patient care services across the continuum, ensuring seamless transitions and optimal patient outcomes.

Key Responsibilities
  • Coordinate patient care services across the continuum, including inpatient, outpatient, and community-based settings.
  • Develop and implement comprehensive care plans that address individual patient needs.
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
Requirements
  • Experience as a Registered Nurse in a Med/Surg setting required.
  • Licensed RN in the state of Massachusetts.
  • Case Manager certificate or equivalent experience preferred.
  • Strong analytical and problem-solving skills, with the ability to work effectively in a fast-paced environment.

Salary Information

The estimated salary range for this position is $92,000 - $123,000 per year, depending on experience.
