The Primary Nurse is a vital member of the EverHeart Hospice team, responsible for providing high-quality patient care and support to individuals and their families. This role requires a compassionate and skilled nurse who can work independently and as part of a team to deliver exceptional care.
Key Responsibilities- Direct Patient Care: Provide skilled nursing care to patients in their homes, including routine homecare visits, end-of-life care, and referrals and admissions.
- Plan of Care: Develop, implement, and revise individualized plans of care in collaboration with patients, families, and healthcare teams.
- Communication: Maintain open and effective communication with patients, families, and healthcare teams to ensure seamless care coordination.
- Education and Training: Provide education and training to patients, families, and caregivers on disease management, symptom control, and end-of-life care.
- Documentation: Maintain accurate and timely documentation of patient care, including progress notes, medication lists, and other relevant information.
- Collaboration: Work collaboratively with other healthcare professionals, including physicians, social workers, and spiritual care coordinators, to ensure comprehensive patient care.
- Quality Improvement: Participate in quality improvement initiatives to enhance patient care and outcomes.
- Education: Graduation from an accredited school of nursing.
- Experience: Minimum of two years of experience in medical-surgical, oncology, or community health nursing.
- Licensure: Current RN licensure in the state of Ohio and eligibility to obtain licensure in the state of Indiana, or current RN licensure in the state of Indiana and eligibility to obtain licensure in the state of Ohio.
- Skills: Strong communication and interpersonal skills, ability to work independently and as part of a team, and proficiency in electronic medical records.
