Key Responsibilities- Coordinate and facilitate successful patient services from pre-admission through discharge.
- Collaborate with the Rehabilitation Program Director and Facility Case Management Director to ensure departmental functions are aligned with patient needs.
- Develop and implement discharge plans that meet the unique needs of each patient.
- Coordinate efforts within the Rehabilitation team to ensure seamless care transitions.
To be successful in this role, you will need:
- Strong medical knowledge and experience in rehabilitation or case management.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong analytical and problem-solving skills.
