Director of Case Management RN

Verovian Healthcare Recruitment Agency is seeking an experienced Director of Case Management RN to lead the case management team in Englewood, Colorado.
Job Summary:The Director of Case Management RN will oversee the coordination of patient care from admission to discharge, ensuring that all patients receive efficient, effective, and high-quality care. This leadership role requires collaboration with physicians, nurses, and social workers to optimize patient outcomes, reduce length of stay, and manage hospital resources effectively.
Key Responsibilities:- Lead and manage the case management department, including utilization review, discharge planning, and care coordination.
- Develop and implement strategies to improve patient flow and reduce hospital readmissions.
- Collaborate with the interdisciplinary team to ensure continuity of care across all phases of treatment.
- Ensure compliance with regulatory requirements, accreditation standards, and hospital policies.
- Monitor departmental performance, set quality and financial goals, and report on key performance indicators.
- Recruit, mentor, and develop case management staff to support the delivery of high-quality care.
- Engage with patients and families to address concerns and provide education about care transitions.
- Current RN license in Colorado.
- 5+ years of nursing experience, with at least 2 years in a case management or leadership role.
- Knowledge of case management, utilization review, and discharge planning processes.
- Strong leadership, communication, and organizational skills.
- Bachelor's degree in nursing (BSN) is required; MSN or related advanced degree preferred.
- Case Management Certification (CCM) or ACM certification is preferred.
