Clinical Care Specialist

We are seeking a highly skilled Nurse Practitioner or Physician Associate to join our team at Optum in Marion County, OR. As a member of our Home and Community Care team, you will provide annual clinical assessments to patients in the comfort of their homes.
Our holistic approach addresses the physical, mental, and social needs of our patients, helping them access and navigate care anytime and anywhere. You will have the ability to achieve work-life balance with flexible scheduling offered where providers can flex their time over the 7-day work week.
About the RoleThis is an exciting opportunity to bring high-end medical service, compassionate care, and industry-leading solutions to our most vulnerable patient populations. As a Clinical Care Specialist, you will be responsible for conducting in-home assessments, performing evidence-based practice screenings, and communicating findings to inform members' PCPs of potential gaps in care.
You will also identify diagnoses, formulate a list of current and past medical conditions, and educate members on topics such as disease process, medication, and compliance. Travel up to 85% may be required.
What We OfferWe offer a competitive salary of $60,000 - $80,000 per year, based on experience. In addition, we offer a Sign-On Bonus of $30,000 for individuals who have not previously participated in this program. Flexible provider schedules available between 24-40 hours per week ensure a great work-life balance.
