Clinical Nurse Specialist

At Drexel University, we are seeking a highly skilled Clinical Nurse Specialist to join our team. As a key member of our Ambulatory Internal Medicine Practice, you will be responsible for ensuring the overall clinical success of our patients. Your primary focus will be on supporting and participating in the Patient-Centered Medical Home (PCMH) concepts of team-based care. This will involve staying up to date with PCMH guidelines, assessing, planning, implementing, coordinating, monitoring, and evaluating healthcare options and services to improve patient outcomes. Your expertise will also be utilized to educate Medical Assistants about PCMH requirements and how to meet them. Additionally, you will collect data on quality metrics and track them as required for improvement efforts. Your collaboration with patients' medical/health providers will be crucial in establishing mutual goal setting, including patients and their families/caregivers, utilizing self-management tools. You will also be responsible for performing blood pressure checks for uncontrolled hypertensive patients, educating uncontrolled diabetic patients on sugar management, monitoring patients' INRs, and assisting patients with access to medical supplies. Your role will also involve coordinating referrals to homecare services, assisting with unstable patients waiting for 911 to arrive, and developing proper clinical procedures to ensure patient safety, comfort, and quality. You will evaluate ongoing patient care through incident reports and enact appropriate actions when necessary and in accordance with all office policies and procedures. As a resource and role model for staff, you will recommend and implement new initiatives when appropriate. You will also review insurance registries to ensure missing gaps are addressed and assure compliance with all Clinic policies and procedures and governmental regulations. Your professional licensure and staying up to date on all DOH, Medicare, AAAHC, and other voluntary and regulatory requirements will be essential. Finally, you will establish and maintain effective working relationships with employees, managers, and physicians, assist with the development of patient education material, and support and promote a positive image of the practice when dealing with patients and others inside and outside the organization.

Required Qualifications

Minimum of a Bachelor's degree in Nursing or Equivalent
Minimum of 3 years of experience

Certification/Licensure:

Current Pennsylvania State Nursing License
CPR certification for Health Care Providers

Preferred Qualifications

Excellent organizational and interpersonal communication skills
Demonstrated ability to manage multiple priorities
Ability to write reports, business correspondence, and letters to patients
Ability to work effectively and respond to questions from groups of patients and physicians

Additional Information
This position is classified as Exempt grade K. Compensation for this grade ranges from $54,630 to $81,940.00 per year. Please note that the offered rate for this position typically aligns with the minimum to midrange of this grade, but it can vary based on the successful candidate's qualifications and experience, department budget, and an internal equity review.
