About the Role:
AuthoraCare Collective is seeking a skilled RN Triage Specialist to join our team on a part-time weekend basis. As a key member of our interdisciplinary team, you will play a vital role in assessing patient needs, assigning visits, and implementing interventions for our agency programs.
Key Responsibilities:
- Triage calls from patients, family members, facilities, and providers to assess needs and assign visits to team members as needed.
- Assess and identify patient/caregiver problems, establish patient/caregiver care goals, and implement interventions for all agency programs.
- Work closely with providers to establish and implement a plan of care for patients.
- Communicate effectively with team members, patients, caregivers, physicians, pharmacists, and other healthcare professionals and community service organizations.
Requirements:
- Current RN licensure in North Carolina or graduation from an accredited School of Nursing.
- One year of nursing experience in triage or case management.
- One year of hospice experience preferred.
- Two years of experience in a triage or case management position preferred.
- Understanding and commitment to the Hospice philosophy of care.
- Excellent assessment and clinical nursing skills, as well as effective interpersonal and communication skills.
- Computer competency required.
Additional Requirements:
- Valid state-issued driver's license required.
- Must carry automobile liability insurance at limits required by the agency.
- Must have own transportation.
