Job Opportunity

We are seeking a skilled and compassionate Nurse Practitioner to join our team at Greater Shores Primary Care. As a key member of our healthcare team, you will play a vital role in providing high-quality, patient-centered care to our community.

Key Responsibilities:

  • Conduct comprehensive patient assessments, including health histories and physical examinations
  • Diagnose and manage acute and chronic conditions using evidence-based practice
  • Collaborate with our healthcare team to develop individualized care plans
  • Prescribe medications and treatments as appropriate, following state regulations
  • Educate patients and families on wellness, preventive care, and chronic disease management
  • Provide routine follow-up care and manage patient health records efficiently
  • Participate in quality improvement initiatives and ensure adherence to clinical best practices
  • Deliver compassionate care with clear and effective communication

Qualifications:

  • Active and unrestricted Nurse Practitioner license
  • Board certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP)
  • Minimum of 2 years of experience in a primary care or outpatient setting (preferred)
  • Strong clinical judgment and ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Proficient with electronic health records and telehealth services
  • Passion for patient education and a holistic approach to healthcare
