As a vital member of the healthcare team, the Clinic RN plays a crucial role in delivering exceptional patient care within the Primary Care department. This position is designed for a dedicated professional who is committed to enhancing patient experiences and outcomes.
Key Details
+ Department: Primary Care
+ Schedule: Full-Time | 40 hours per week | Monday to Friday, daytime shifts with variable hours
Responsibilities
The Clinic RN will:
- Act as a patient advocate, guiding individuals through their healthcare journey.
- Serve as a communication bridge among management, physicians, and community resources.
- Facilitate treatment coordination and support quality improvement initiatives.
- Gather and analyze data to recommend enhancements in care processes.
- Assist uninsured or underinsured patients in accessing necessary follow-up care.
Qualifications
To be considered for this role, candidates must possess:
- Active Registered Nurse licensure from the Wisconsin Board of Nursing or a multi-state/Compact State license.
- Preferred BLS certification from the American Heart Association or American Red Cross.
- A diploma from an accredited nursing program or equivalent professional licensure.
Preferred Experience
+ Degree in Nursing
+ 3-5 years of nursing experience, with a preference for primary care settings
+ BLS Certification

Essential Functions
The RN will:
- Deliver direct patient care with a focus on comprehensive wellness.
- Conduct Medicare Annual Wellness visits and coordinate patient education.
- Proactively address patient concerns and ensure satisfaction.
- Maintain accurate documentation to meet regulatory and billing standards.
- Engage in pre-visit planning to optimize patient care and follow-up.
- Understand and apply preventive care guidelines to enhance patient health.
- Connect patients with community resources to support their healthcare needs.
