Job Title: Nurse Practitioner or Physician Assistant

Optum is seeking a skilled Nurse Practitioner or Physician Assistant to join our Home-Based Medical Care team in Buffalo, Orchard Park, W. Seneca, Hamburg, Amherst, and Cheektowaga, NY.

About the Role:

As a member of our interdisciplinary team, you will provide high-quality, patient-centered care to complex, chronic patients in their homes. You will work closely with our healthcare professionals to develop and implement individualized care plans, ensuring the best possible outcomes for our patients.

Responsibilities:
  • Conduct comprehensive assessments of patients to identify their needs and develop personalized care plans.
  • Provide routine and urgent medical care to patients in their homes, including medication management, wound care, and disease management.
  • Collaborate with our healthcare team to coordinate care and ensure seamless transitions between healthcare settings.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal care.
  • Participate in quality improvement initiatives to enhance patient outcomes and care delivery.
Requirements:
  • Current state RN license (if Nurse Practitioner) or NP or PA license.
  • Completion of a Nurse Practitioner or Physician Assistant accredited program.
  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA).
  • Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems.
Preferred Qualifications:
  • 2+ years of clinical experience.
  • Knowledge of CPT, ICD-9, and HCPC codes.
  • Knowledge of clinical standards of care.
  • Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems.
