Position Overview

Role Summary

The Registered Nurse plays a pivotal role in case management, overseeing a designated group of patients, generally within a specific geographic area. This position is accountable for the comprehensive management of post-acute services delivered in a home environment. The RN provides direct patient care while supervising LPNs and HHAs, adhering to a pre-established care plan tailored to meet individual patient needs, goals, and physician directives. The role involves coordination of care with a multidisciplinary team and serves as the primary liaison for patients with other healthcare providers throughout their care continuum. The RN ensures that patient care is appropriate, qualified, and timely, fulfilling the needs of patients while aligning with the agency's financial, operational, and quality objectives.

Key Responsibilities

  • Manage an assigned patient caseload daily, ensuring timely delivery of care and documentation in accordance with the established care plan and physician orders.
  • Gather relevant health data from patient assessments conducted in the home, telephonic interactions, and available EMR records.
  • Evaluate collected data to identify issues and resources for effective resolution. Recognize patients at elevated risk for re-hospitalization, falls, or high resource utilization, necessitating further intervention and case discussions to ensure quality outcomes.
  • Prioritize complex situations based on the urgency of patient health, safety, and the effectiveness of service delivery.
  • Identify and address barriers to resolution (such as language, literacy, or psychological factors) by engaging appropriate resources or leadership for timely intervention.
  • Develop and oversee a care plan that is clearly communicated among all parties involved, promoting quality patient outcomes and satisfaction.
  • Implement the care plan and direct the coordination of a multidisciplinary clinical team, supervising LPNs and/or HHAs involved in the care of the assigned patient group to ensure safe and effective outcomes.
  • Administer prescribed treatments, therapies, and procedures in compliance with the State Nurse Practice Act, Home Health Conditions of Participation, JCAHO standards, and agency policies.
  • Assess progress towards achieving patient-centered goals.
Physical Requirements
The role necessitates walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling. The RN must be capable of lifting and supporting up to 35 pounds when handling patients, medical equipment, and supplies. Proficiency in English communication is required. The position demands intact sensory functions, including sight, hearing, smell, and touch, along with finger dexterity. Critical thinking and the ability to maintain focus are essential, as is the capacity to respond swiftly to changes in patient conditions. Physical Abilities Testing is a requirement.

Qualifications

Candidates must be graduates of an accredited School of Nursing, with a BSN preferred. BLS certification is mandatory per policy guidelines. A current RN license or a temporary license as a Registered Nurse Petitioner in the state of practice is required. For those declaring a National License Compact (NLC) state as their primary residency, compliance with home state licensure requirements is necessary. Additional education, training, certifications, or experience may be mandated within the department by the nursing leadership.
