Home Health Therapist Position

Kaiser Permanente, a leading not-for-profit health organization, is seeking an experienced Home Health Physical Therapist for a role in Lancaster, California.
Job Overview- Average Salary: $120,000 - $150,000 per year
The selected candidate will provide independent physical therapy to patients in their place of residence, teaching preventative and rehabilitative procedures to enhance patient care. A strong background in developing and coordinating treatment plans is essential.
Key Responsibilities- Evaluate patients comprehensively to determine the nature and extent of dysfunction, using medical assessments of skin integrity and mentation to detect early warning signs of complications.
- Establish treatment goals and plans treatment to achieve established objectives, considering the patient's holistic needs beyond just body part or functional ability.
- Initiate discharge planning for physical therapy, ensuring a smooth transition for patients upon completion of treatment.
- Implement the plan of care through direct treatment of the patient and other healthcare personnel as necessary, providing advice and education to patients and families on physical therapy management.
- Regularly reassess clinical symptoms to determine the effectiveness of treatment, progress towards goals, and the need for modifying treatment plans.
- Discharge patients from physical therapy when they have met their treatment objectives.
- Maintain accurate and timely documentation of physical therapy patient management, adhering to guidelines and regulations.
- Participate in approved educational and clinical research activities in physical therapy, staying up-to-date with industry developments.
- Supervise the work of Home Health Aides and Physical Therapy Assistants according to regulation and agency policy.
- Minimum of two (2) years experience in physical therapy.
- Graduate of an accredited physical therapy curriculum with a Bachelors, Masters, or Doctoral degree and certification in Physical Therapy or foreign-trained equivalent.
- Physical Therapist License (California) and Basic Life Support certification.
- Driver's License (California) and National Provider Identifier.
