Home Health Physical Therapist

Kaiser Permanente is seeking a skilled Home Health Physical Therapist to join our team in Lancaster, California. As a key member of our home health care team, you will provide high-quality physical therapy services to patients in their place of residence.
About the Role:
This full-time position offers an excellent opportunity to work with a dynamic team and make a meaningful impact on the lives of our patients. The successful candidate will be responsible for providing independent, age-appropriate physical therapy to patients, detecting early warning signs of complications, and initiating discharge planning. You will also implement the plan of care through direct treatment of the patient and other healthcare personnel as appropriate.
Key Responsibilities:
- Provide independent, age-appropriate physical therapy to patients in their place of residence
- Detect early warning signs of complications, including medical assessments of skin integrity and mentation
- Initiate discharge planning for physical therapy
- Implement the plan of care through direct treatment of the patient and other healthcare personnel as appropriate
- Discharge patients from physical therapy when they have met their treatment goals
- Provide accurate, timely documentation of physical therapy patient management
- Participate in appropriate and approved educational and clinical research activities in physical therapy
- Coordinate with pharmacy, inpatient physical therapy, outpatient departments
- Assist in the development, implementation, and review of departmental patient care programs
- Perform other duties as needed in the areas of staff education, peer reviews, departmental operations, and departmental projects
- Supervise the work of Home Health Aides and Physical Therapy Assistants according to regulation and agency policy
Requirements:
To be successful in this role, you will need:
- A minimum of two (2) years experience in physical therapy
- Graduate of an accredited physical therapy curriculum with a Bachelors, Masters, or Doctoral degree with certification in Physical Therapy or foreign-trained equivalent
- Clinically skilled to detect early warning signs of complications, including medical assessments of skin integrity and mentation
- Effective written and oral communication skills (in English)
- Ability to work in a Labor/Management Partnership environment
- 1-year experience in home health agency preferred
Salary:
The salary for this position is estimated to be between $83,000 - $110,000 per year, depending on experience. Benefits include competitive pay, comprehensive benefits package, and opportunities for professional growth and development.
