Medication Care Partner

We are seeking a compassionate and detail-oriented Medication Care Partner to join our team at Tribute at One Loudoun. As a Medication Care Partner, you will play a vital role in providing direct care to our senior residents, ensuring their medication and treatment plans are followed, and promoting a positive and supportive living environment.
Key Responsibilities:
- Administer medication to residents in accordance with their personalized medication and treatment plans.
- Keep accurate and up-to-date charting documentation, reporting any changes in resident status.
- Coordinate medication orders and deliveries with pharmacies, ensuring timely and accurate delivery.
- Provide emotional support and companionship to residents, encouraging independence and participation in life enrichment activities.
- Promote open communication between healthcare professionals, families, and staff, ensuring a collaborative approach to resident care.
Requirements:
- Current state Certified Medication Tech (CMA) license in VA.
- Current caregiver license per state requirements (CNA/GNA/PCA/DCA license).
- First Aid and CPR certification or ability to obtain.
- Previous experience working in Independent Living (IL), Assisted Living (AL), Memory Care (MC), or Senior Living is a plus.
- High School Diploma or equivalent.
