The Clinical Pharmacist is a key member of the healthcare team, responsible for providing high-quality, individualized pharmaceutical care and integrated pharmacy operational services to optimize patient medication therapy outcomes and quality of life.
Key Responsibilities- Develop and implement patient-centered medication therapy plans in collaboration with the healthcare team.
- Conduct thorough medication reviews and provide recommendations for optimal medication use.
- Collaborate with physicians and other healthcare professionals to ensure seamless patient care.
- Provide education and support to patients and their families on medication use and management.
- Stay up-to-date with the latest clinical guidelines and research to ensure evidence-based practice.
- PharmD degree or equivalent.
- California Pharmacist Registration upon hire.
- Basic Life Support-Provider (BLS) certification.
- Preferred qualifications: Acute care hospital pharmacy experience, ASHP-accredited PGY1 residency completion.
