Job Summary:

This role provides administrative and operational support to population health initiatives, focusing on improving overall health outcomes for patients.

\

Responsibilities:

  • Program Coordination:\
    • \
    • Coordinate meetings, events, and training sessions related to population health management.\
    • Assist in updating and maintaining program documentation, including policies, procedures, and guidelines.\
    • Process incoming referrals from member providers and stakeholders, assigning to appropriate team members.\
    • Triage incoming calls for coordination, routing to appropriate team members and ensuring timely follow-up while maintaining excellent customer service.\
    • Update population health program resources and documents, ensuring they are accurate and available. Archive outdated documents.\\
    • Data Management:\
      • \
      • Collect, organize, and manage health-related data from various sources.\
      • Assist in analyzing data to identify trends, gaps, and opportunities for improvement.\
      • Assist in preparing reports and presentations based on data analysis for stakeholders.\
      • Utilize multiple software programs, including care management platforms, electronic medical records (EMR), Microsoft Office applications, and payor databases.\\
      • Communication and Outreach:\
        • \
        • Support the development and dissemination of communication materials, such as newsletters, brochures, and educational content.\
        • Assist in coordinating outreach efforts to engage community members, healthcare providers, and other stakeholders.\
        • Respond to inquiries and provide information about population health programs to internal and external audiences.\\

Requirements:

  • \
  • Associates degree preferred with minimum 3 years' experience in a health-related field.\
  • Minimum 2 years of successful operational support experience in healthcare or with a proven track record in a similar role.\
  • Knowledge of medical terminology, medical coding, and/or experience working in a clinical setting.\
  • Knowledge of HIPAA, including Protected Health Information (PHI), and other health-related compliance regulations.\\

Estimated Salary Range: $70,000 - $90,000 per year
