Job Description:
We are seeking a skilled Physical Therapist to join our team in Houma, LA. As a Home Healthcare Physical Therapist, you will be responsible for helping patients maintain, restore and improve movement, activity and function. Your interventions will focus on conducting reexaminations (every 30 days, at minimum), modifying interventions as necessary, and performing discharge planning. Other duties may include providing patient education and support, collaborating with healthcare teams, and participating in quality improvement initiatives.

Required Skills and Qualifications:
To succeed in this role, you must have a current license to practice physical therapy specific to the state of Louisiana, a graduate degree from an accredited physical therapy educational program, and one year of related experience in home health care or a similar setting. You should also possess strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
