Key Responsibilities:
- Develop and Implement Treatment Plans: Collaborate with interdisciplinary teams to create comprehensive care plans that meet the unique needs of each patient.
- Provide Education and Support: Educate patients and their families on various aspects of health and wellness, including disease management, medication adherence, and healthy lifestyle choices.
- Contribute to Quality Improvement Initiatives: Participate in quality improvement projects aimed at enhancing patient outcomes, reducing readmissions, and improving overall healthcare delivery.
- Maintain Accurate Records: Document patient information accurately and thoroughly, adhering to HIPAA guidelines and maintaining confidentiality.
- Stay Current with Industry Developments: Engage in ongoing education and training to stay abreast of best practices, new technologies, and emerging trends in healthcare.
Requirements:
- Active RN License: Possess an active RN license in the state of Pennsylvania or multi-state licensure through the eNLC.
- Certifications and Training: Maintain current certifications in CPR and any relevant specialty areas, such as trauma or pediatrics.
- Strong Communication Skills: Demonstrate excellent written and verbal communication skills, with the ability to articulate complex medical concepts to patients and colleagues alike.
- Ability to Work Effectively in a Team Environment: Collaborate with nurses, physicians, and other healthcare professionals to ensure comprehensive care and address complex patient issues.
Salary Range: $62,000 - $80,000 per year, depending on experience.
