Benchmark Senior Living is seeking a compassionate and dedicated Licensed Practical Nurse (LPN)/Registered Nurse (RN) to join our team at Putnam Farms.

Job SummaryWe are seeking an LPN/RN with current state licensure and CPR certification to provide exceptional care to our residents. Your primary responsibilities will include assessing, communicating, and documenting resident status, observing and reporting changes in behavior and health, directing daily work assignments of Resident Care Assistants, and coordinating care needs with community providers.
You will have the opportunity to work closely with our interdisciplinary team to develop personalized care plans that promote resident independence and overall well-being.
Responsibilities- Assessment and Documentation: Accurately assesses, communicates, and documents residents' status, including any significant changes in behavior or health.
- Care Coordination: Directs and supervises the daily work assignments of Resident Care Assistants and coordinates care needs with community providers via an effective case management process.
- Leadership and Mentorship: Closely mentors, guides, and directs the Lead Resident Care Assistants in all aspects of their job, promoting a culture of teamwork and collaboration.
Additionally, you will respond to emergencies and personal emergency response system, recognizing and assisting in the prevention of elder abuse, neglect, and exploitation.
Requirements- Current state license as an LPN and CPR certification.
- Graduate of an approved LPN program (per state requirements).
- Minimum of 1 to 2 years of experience working as a nurse preferred.
- Previous experience working with the elderly in a paid or volunteer position is highly desirable.
$60,000 - $80,000 per year, depending on experience and qualifications.
