**Job Title:** Registered Nurse Case Manager

i4 Search Group seeks a skilled Registered Nurse to join our team as a Case Manager. This rewarding role involves working closely with patients, healthcare professionals, and family members to provide comprehensive care and support.

Annual Salary Range: $90,000 - $120,000

About the Position:

We are looking for a highly motivated and empathetic individual to fill this critical role. As a case manager, you will play a vital part in ensuring that patients receive the highest level of care possible. Your duties will include assessing patient needs, developing personalized care plans, and coordinating services to meet those needs.

Main Responsibilities:

  • Screening, assessment, planning, problem resolution, resource management, counseling, and crisis management for patient/family health care and discharge planning needs
  • Collaboration with interdisciplinary teams to coordinate care and resolve patient needs

Essential Qualifications:

  • Active State Registered Nursing license
  • Accredited nursing degree
  • Minimum two years of experience in a clinical setting, with preference given to candidates with case management experience
  • Valid BLS certification
  • Able to work effectively in a dynamic healthcare environment
