Job Title: Patient Support and Care Coordinator

Estimated Salary: $45,000 - $65,000 per year.

We are seeking a caring and empathetic Patient Support and Care Coordinator to join our team at BeLoved Hospice, Inc. This is a fulfilling opportunity for an individual who is passionate about delivering exceptional patient care and support to those in need.

About the Role:

The successful candidate will work closely with our multidisciplinary team to provide comprehensive patient care and support. Key responsibilities include:

  • Providing personalized care to patients, including assistance with daily living activities, vital sign taking, and documentation of patient information.
  • Collaborating with the RN Case Manager to develop and implement patient care plans.
  • Participating in palliative care and pain management within the scope of practice.
  • Providing emotional support and companionship to patients and their families.

Requirements:

  • Current certification as a Certified Nurse Aide by the Oregon Board of Nursing.
  • Prefer one year experience as a CNA in hospice or home health or geriatric care.
  • Reliable transportation and auto insurance.
  • CPR/First Aid certification.
