Job Summary

The Clinical Program Specialist at Roots Community Health Center plays a vital role in implementing programs for high-priority conditions such as HIV, Hepatitis C, Opioid Use Disorder, and Diabetes. This position requires exceptional client/patient service skills, strong communication, and the ability to work effectively with diverse stakeholders.

Key Responsibilities
  • Develop and maintain client/patient tracking systems to ensure timely and effective coordination of care.
  • Collaborate with patients' navigation team members to coordinate care and share patient progress.
  • Coordinate referrals for patients as needed, ensuring seamless transitions between services.
  • Work with the patient's care providers to support recommended visit schedules for clinical programs.
  • Provide education and outreach to patients about clinical programs, including PrEP, MAT, and HCV treatment.
  • Attend regular program meetings to discuss service coordination, challenges, and barriers to conducting the work.
Requirements
  • Minimum 1 year's experience performing clinical administrative functions.
  • Interest/experience working with low-income communities of color.
  • Excellent written and verbal communication skills.
  • Ability to manage multiple phone lines, spreadsheets, and files.
  • Ability to work with patients from diverse backgrounds.
  • Strong working knowledge of Microsoft Office and G-Suite.
