Home Health Intake Licensed Practical Nurse (LPN) Opportunity

As a Home Health Intake Licensed Practical Nurse (LPN), you'll play a vital role in our team, ensuring seamless communication with referral sources, outside agencies, and health professionals.

Key Responsibilities:
  • Act as the primary point of contact for patients, families, and referral sources, providing accurate information and assistance.
  • Coordinate incoming calls, ensuring timely responses and resolving any issues that may arise.
  • Collaborate with clinical staff to adhere to standards of practice, laws, and regulations.
  • Collect and enter customer information into our management information system.
  • Review and enter orders from referral sources, maintaining accurate records.
Requirements:
  • Graduate of an accredited practical nursing program and active LPN license in Colorado.
  • Minimum of one year of LPN experience, preferably in home healthcare.
  • CPR certification.
  • QAPI experience preferred but not required.
  • Able to perform home visits on a PRN basis.
  • Pass a background check and drug test.
  • Knowledge of state and federal home health regulations.
  • Clinical proficiency in nursing, strong communication skills, and compassion.
