Home Health Intake Licensed Practical Nurse (LPN) Job Description

As a Home Health Intake Licensed Practical Nurse (LPN) with Interim HealthCare, you will play a vital role in providing exceptional patient care while enjoying the flexibility and autonomy that comes with this role.

Key Responsibilities:
  • Act as the primary contact for referral sources, outside agencies, and health professionals to coordinate patient care and services.
  • Communicate with patients and families to arrange for care and services, ensuring a seamless experience.
  • Collect and enter customer information into our management information system, ensuring accurate and timely data.
  • Work collaboratively with clinical staff to adhere to standards of practice for nursing and applicable laws and regulations.
  • Receive and review orders from referral sources, entering information into the management information system.
Requirements:
  • Graduate of an accredited practical nursing program and active LPN license in Colorado.
  • Minimum of one (1) year of LPN experience, preferably in home healthcare.
  • CPR certification.
  • QAPI experience preferred but not required.
