Social Work Case Manager - PRN

Job Summary:

We are seeking a skilled Social Work Case Manager to join our team at LifeBridge Health. As a key member of our healthcare team, you will be responsible for partnering with patients, families, and healthcare providers to ensure efficient care and effective management of patient needs.

Responsibilities:

  • Coordinate with community agencies to address abuse, neglect, substance abuse, mental health, or adoption
  • Link patients to financial resources and other support services
  • Collaborate with healthcare teams to develop and implement patient care plans
  • Provide education and support to patients and families

Requirements:

  • Master's degree in Social Work required
  • 2-4 years of Social Work experience required
  • Recent Social Work experience in Medical Social Work preferred
  • LCSW-C - Certified Clinical Social Worker required
