Job Summary

We are seeking a skilled Registered Nurse PRN to join our Home Health team in Inverness. As a PRN RN, you will provide high-quality nursing care to patients in their homes, working closely with physicians and other healthcare professionals to develop and implement individualized care plans.

Key Responsibilities:

  • Clinical Competence
    • Initiate, develop, and implement comprehensive care plans in collaboration with physicians and other healthcare professionals.
    • Supervise and direct the care provided by home health aides and licensed practical vocational nurses, ensuring adherence to state and federal regulations.
  • Documentation and Care Delivery
    • Provide high-quality clinical services within the scope of practice, adhering to infection control standards and payer requirements.
    • Accurately and timely complete comprehensive assessments, including medication reconciliation, and document patient visits according to policy and payer requirements.
  • Quality
    • Ensure patients meet home health eligibility and medical necessity guidelines, accurately determining primary focus of care and developing comprehensive care plans with physicians.
    • Communicate effectively with agency staff, physicians, patients, and family members to ensure quality care and service excellence.
  • Professionalism
    • Display a positive attitude, willingness to help patients and the agency, and openness to learning new skills.
    • Be punctual for meetings and actively participate in case conferences, including video meetings.
  • Teamwork
    • Take direction from Clinical Director and Executive Director professionally, completing tasks assigned timely and assisting in the orientation of new agency personnel.
    • Participate in survey readiness activities, performance improvement plans, and implement processes to ensure positive patient outcomes.

Requirements:

  • Current RN licensure in the state of practice
  • Current CPR certification
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
