VNS Health is seeking a skilled Social Work professional to join our team in Manhattan, New York.

Job Summary

We are looking for a highly motivated and experienced Social Worker to provide care management services to our members. The ideal candidate will have a strong background in social work, excellent communication skills, and the ability to work collaboratively with interdisciplinary teams.

Key Responsibilities
  • Assess and develop comprehensive care plans that address the unique needs of our members.
  • Provide intensive and continuous care management across acute, home, and long-term care settings.
  • Collaborate with primary care practitioners, interdisciplinary teams, and family members to ensure optimal delivery and coordination of services.
  • Monitor care management activities, services, and members' responses to interventions to determine the effectiveness of the plan of care.
  • Identify trends and needs of groups in the community and plan interventions based on these identified needs.
Requirements
  • Master's Degree in Social Work required.
  • Minimum of three years of Social Work experience required.
  • License and current registration to practice as a Licensed Social Worker in New York State preferred.
  • Case Management Certification preferred.
