Responsibilities

The Speech Language Pathologist is responsible for delivering speech therapy services as part of comprehensive patient care. Key responsibilities include:

  • Conducting assessments and evaluations to identify patient needs and collaboratively establishing goals for the care plan.
  • Providing speech therapy services, including assessment, evaluation, procedures, teaching, and training activities as outlined in the care plan.
  • Reporting patient progress to the Clinical Supervisor and referring Physician.
  • Documenting daily updates and changes to patient care plans, including discharge planning.
  • Participating in staff meetings, team conferences, and educational in-services as required.
  • Performing all OASIS time point assessments in accordance with Medicare criteria and submitting recertification documentation as per policy.
Qualifications
  • A current, unencumbered State Professional License.
  • A valid driver's license and proof of insurance.
  • A minimum of one year of experience as a professional Speech Language Pathologist in a home care setting, with competency in comprehensive assessment and OASIS-C.
  • The ability to exercise sound clinical judgment.
  • Proficiency in assessing and documenting patient needs and formulating individualized care plans.
  • Strong clinical skills.
  • Proficiency in Microsoft Office applications.
