Job Summary

The Hospice and Palliative Care Nurse is a critical member of our interdisciplinary team, providing care to patients with terminal or life-limiting conditions and their families in their home setting. This role requires a compassionate and competent approach to supporting patients and families, with a focus on pain, symptom, and plan of care management. The nurse will work closely with the team to develop and implement individualized care plans, and will be responsible for performing home-based care with a family-centered approach. The ideal candidate will have excellent critical thinking skills, good communication and customer service skills, and the ability to work independently and as part of a team.

Responsibilities

Perform home-based care with a family-centered approach
Act as the on-call nurse for a portion of each month, divided among the nursing team
Practice with a competent and compassionate approach to supporting patients and families facing terminal and life-limiting conditions
Actively participate in the interdisciplinary team and maintain their primary patients' plan of care

Qualifications

BSN Required
Ohio Driver's License
Excellent critical thinking skills, good communication and customer service skills
Ability to organize and manage time well
Ability to work independently, as well as within an interdisciplinary team
Other necessary skills include, but are not limited to, pediatric assessment, infusion and central line care, injections, wound care and patient education
