It is imperative to comply with all institutional policies and protocols.Key Responsibilities:
1. Collaborate with interdisciplinary teams to optimize pharmaceutical care.
2. Review and interpret medication orders and prescriptions for inpatients, ensuring correct dosage, administration route, frequency, and screening for potential interactions and allergies.
3. Deliver clinical pharmacy services to hospitalized patients, including order verification, medication reconciliation, pharmacokinetic dosing and monitoring, renal dose adjustments, anticoagulation management, and IV to oral medication conversions.
4. Execute medication distribution functions, which include preparation and verification of medications, labeling, compounding, and dispensing.
5. Provide drug information and support to prescribers, hospital staff, and patients.
6. Uphold established procedures for quality assurance, security of controlled substances, and safe disposal of hazardous medications.
7. Supervise pharmacy technicians, providing necessary guidance and performance oversight.
8. Participate in the training and orientation of pharmacy technicians, residents, pharmacists, and other healthcare professionals.
9. Adhere to the regulations and standards set forth by pharmacy governing bodies and the hospital.
10. Prepare a diverse range of sterile products and perform pharmaceutical calculations for various formulations.
11. Ensure the safe handling, preparation, storage, and disposal of cytotoxic and hazardous drugs.
12. Maintain strict adherence to sterile procedures and ensure proper operation and maintenance of pharmacy equipment.
13. Review physician orders for intravenous solutions, parenteral orders, inpatient unit dose orders, and bulk drug orders.
14. Conduct final checks on sterile products, bulk drug orders, and emergency medication replenishments according to local policies.
15. Perform advanced pharmaceutical calculations for the preparation of intravenous admixtures and complex parenteral nutrition solutions.
16. Enter medication orders into the inpatient computer system, reviewing for allergies, drug interactions, and duplicate orders.
Qualifications:
- Minimum of 1 year of experience in a hospital pharmacy setting.
- Current certification is required.
- PharmD degree is mandatory.
- Experience in IV compounding is essential.
Job Type:
Contract (40 hours/week) with variable hours.
