Job Summary:

As a skilled Home Health RN, you will provide high-quality nursing care to patients in their own homes, working under the direction of a physician's plan of care. This role requires a strong commitment to patient-centered care, with a focus on promoting independence and improving quality of life.

Key Responsibilities:

• Develop and implement individualized plans of care that address the patient's holistic health needs

• Provide skilled nursing services, including wound care, medication management, and vital sign monitoring

• Collaborate with patients, families, and healthcare teams to ensure seamless care coordination

• Educate patients and families on self-care techniques, disease management, and health promotion

• Conduct thorough assessments to identify patient needs and develop realistic, measurable outcomes

Requirements:

• Current R.N. license in the State of Maine

• ANA certification in Home Health or Community Health Nursing (encouraged)

• Current CPR card required

• One year of recent clinical nursing experience

• Reliable car with insurance
