Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at IntelyCare. As an LPN, you will have the opportunity to work in post-acute facilities, tracking the wellbeing of patients and monitoring their vital signs. You will also be responsible for conducting intakes, documenting health concerns and medical history of residents, administering and verifying medications, and reviewing residents' charts and medical records.

Responsibilities
  • Work in post-acute facilities as an LPN
  • Track the wellbeing of patients, both physical and mental, and monitor their vital signs
  • Conduct intakes, document health concerns and medical history of residents
  • Administer and properly verify medications and monitor their effects
  • Review residents' charts and medical records
  • Assist with the cultivation of a safe, comfortable facility
  • Maintain appropriate stocks of medical supplies
  • Participate in IV set up, immunizations, and phlebotomy procedures
  • Help with the orientation and training of medical staff
Qualifications and Skills
  • Current New Jersey state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of approved Licensed Practical Nurse (LPN) educational program
  • High school diploma (or GED/equivalent)
  • At least 1 year of experience (LPN or related role)
