Job Description

We are seeking an experienced Occupational Health and Support Coordinator to join our team. In this role, you will collaborate with Managed Care Organizations to support individuals aging in place who require home and community-based services.

Main Responsibilities:
  • Conduct comprehensive assessments to identify participants' needs, including consideration of health history, functional abilities, environmental safety, and social support.
  • Collaborate with MCO representatives to make recommendations regarding participant needs and provide education on options for home modifications, DME, assistive technology, or other adaptive equipment.
  • Educate program participants and MCO representatives on the home and community-based service system.
  • Maintain working knowledge of home and community-based services, MCO processes, and state regulations.
  • Meet productivity standards and metrics benchmarks.
Required Skills and Qualifications:
  • Unrestricted OTA PTA, OT, or PT license.
  • Fluent in Gujarati.
  • 2+ years working in social service or healthcare-related field.
  • Experience working with people with disabilities and seniors.
  • Knowledge of the home and community-based service system.
  • Proficiency with reviewing and interpreting medical history.
