About the Job:

We are seeking a highly motivated and compassionate Case Manager to join our team at Family Service League Inc. The successful candidate will be responsible for working with clients who have behavioral, medical, and/or substance abuse disorders, providing intensive support and services to help them achieve their goals and improve their overall well-being.

About the Role:

  • Develop and implement individualized service plans to address clients' unique needs and goals.
  • Collaborate with clients' healthcare providers to ensure coordinated care and optimal outcomes.
  • Conduct regular assessments and evaluations to monitor clients' progress and adjust service plans as needed.
  • Maintain accurate and timely records of client interactions, including progress notes, treatment plans, and discharge summaries.
  • Participate in team meetings and training sessions to stay current on best practices and new developments in the field.

Requirements:

A bachelor's degree in a health-related field and a minimum of 2 years of experience providing direct services to clients with complex needs are required. Proficient computer skills, including Microsoft Office, are essential. Experience with electronic health records (EHRs) is preferred. Strong communication and interpersonal skills, as well as the ability to work independently and as part of a team, are also essential.
