About the Role

We are seeking a highly skilled Clinical Pharmacist to join our team at Providence Facey Medical Group. As a Clinical Pharmacist, you will play a critical role in providing high-quality, patient-centered care to our ambulatory patients.

Key Responsibilities
  • Direct Patient Care/Population Health:
    • Provide collaborative drug therapy management (CDTM) in designated Facey services.
    • Perform pharmaceutical assessment and reassessment of patient medication regimen.
    • Act as a consultative resource to physicians and other members of the health care team.
    • Coordinate the pharmaceutical care of patients.
    • Develop and execute care plans tailored to individual patient needs.
    • Analyze lab results and provide clinical monitoring.
    • Order laboratory tests as necessary.
    • Partner with the care team to enhance patient care delivery.
  • Administrative/Leadership/Professionalism:
    • Deliver exceptional service to patients and fellow staff members.
    • Respect patient dignity, confidentiality, and privacy.
    • Work in a safe manner and report any unsafe conditions.
    • Exhibit behavior and communication skills that demonstrate Facey's commitment to superior customer service.
    • Establish effective working relationships with Facey physicians and staff.
  • Educator/Preceptor:
    • Provide education to Facey providers, staff, and patients.
    • Assist in clinical instruction and supervision of medical assistants and pharmacy students.
    • Participate in collaborative disease state education programs.
Requirements
  • Graduate from an accredited school of pharmacy.
  • PharmD.
  • California Pharmacist Registered upon hire.
  • Three (3) years experience with direct patient contact in acute or ambulatory care setting.
Preferred Qualifications
  • Residency in ambulatory or managed care programs.
  • Chronic disease state knowledge base and related pharmaceutical counseling and/or motivational interviewing experience.
  • EHR experience (interpreting and inputting data).
