Job Summary

We are seeking a skilled LPN II to join our team in Skilled Nursing. As a key member of our team, you will provide high-quality patient care, work collaboratively with our interdisciplinary team, and contribute to the overall success of our organization.

Responsibilities
  • Provide direct patient care, including administering medications, performing treatments, and documenting patient information.
  • Collaborate with our interdisciplinary team to develop and implement patient care plans.
  • Contribute to the overall success of our organization by participating in quality improvement initiatives and maintaining a safe and clean work environment.
Requirements
  • High School Diploma/GED (required)
  • Two years of recent LPN experience (preferred)
  • Licensed Practical Nurse (required)
  • Basic Life Support (required)
  • State IV Certification upon hire or within three months of hire/transfer (required)
