Job Description:

We are seeking a skilled Home Health Support Specialist to join our team at 365 Health Services. As a Home Health Support Specialist, you will provide high-quality care to patients in their homes, assisting with daily activities and personal care needs.

About the Role:

  • Perform or assist patients with personal care activities as prescribed in the plan of care.
  • Practice Universal Infection Control Precautions in all contacts with patients.
  • Provide assistance with home maintenance activities as prescribed in the care plan.
  • Respect patient autonomy and confidentiality at all times.

Responsibilities:

  • Document all care provided and submit documentation in a timely manner.
  • Ensure compliance with local, state, and federal laws, Medicare regulations, and agency policies.
  • Meet or exceed established service standards.
  • Maintain regular attendance and adhere to agency policies.

Benefits:
Competitive salary: $45,000 - $65,000 per year based on experience, benefits package including health, dental, and vision insurance, flexible schedule, referral program, and more.
