Job SummaryWe're seeking a skilled Nurse to join our team as a Clinical Care Coordinator. In this role, you'll play a vital part in coordinating patient care and physician access, ensuring seamless communication between patients, staff, and physicians.
Key Responsibilities:- Assist physicians and staff with daily patient care coordination and access.
- Function as a liaison between patients, staff, and physicians, maintaining efficient patient flow.
- Deliver direct patient care within the scope of licensure and in accordance with our policy.
- Collect lab specimens, accurately label them, and prioritize patient care by implementing physician orders and nursing procedures.
- Effectively communicate problems, concerns, or issues to the Office Supervisor/Manager promptly.
- Ensure proper preparation for patient visits, preparing pertinent information needed for patient care.
- Employ appropriate and timely use of Tasking in EHR.
- Obtain vital signs, administer injections, and demonstrate telephone triage protocols.
- Schedule appointments, adhere to universal safety precautions, and review medication lists in EHR.
- Provide referrals and/or consult orders, check and maintain logs for emergency equipment, and facilitate transition of care to UCC/Hospital.
- Maintain proficiency with department-specific equipment, troubleshoot, and report malfunctions promptly.
- Administer chemotherapy, immunotherapy, antibiotics, symptom management, and patient education, coordinating patient care activities with other healthcare team members.
- Associate degree required; Bachelor's degree preferred.
- Oregon State Nursing License and BLS certification required.
- 0-1 year's experience; 2-4 years experience preferred.
- Ability to communicate in English, both orally and in writing.
- Strong interpersonal and organizational skills.
- Ability to perform diverse work assignments with time limitations with a high degree of accuracy.
- Ability to use problem-solving, critical thinking, and priority-setting skills.
