Case Manager Specialist

">Job Summary:The Case Manager will work under the direction of the Program Director to provide case management services to clients in various housing programs. The successful candidate will be responsible for assisting clients with finding permanent housing, preparing and submitting housing applications, and performing a variety of office and field activities as needed.
">Responsibilities:- Work with CES for referrals of eligible clients
- Manage a caseload of 10-20 clients
- Assist homeless and formerly homeless individuals to become independent and successful by assisting in obtaining permanent housing and providing follow-up case management services
- Document and track all client meetings, interactions, and information in the HMIS Database
- Maintain complete and accurate files
- Conduct weekly meetings with clients that focus on independent housing placement and guiding them through the application process
- Assist clients in obtaining and maintaining mainstream benefits (Food Stamps, Health Insurance, SSI/SSDI) if/when applicable
- Obtain resources and services as requested and/or needed to address barriers to housing
- Develop partnerships and collaborative relationships with internal and external service providers to ensure clients receive comprehensive services
- Establish and monitor mental health supports and substance abuse recovery
- Make referrals as needed to substance abuse treatment centers, mental health agencies, vocational/educational resources, legal services, etc.
- Respond to clients who may be in a mental health or other crisis
- Participate in on-call weekly schedule
- Participate in individual and group supervision as assigned by Program Director
- Complete monthly inspections of apartments, dorms, or rooms depending on the program
Qualifications:
- Bachelor's Degree or experience in the field accepted
- 1-3 years' experience; preferably working with homeless adults with substance and illicit drug abuse issues
- Knowledge of low-income housing in Rhode Island as well as special needs housing, including Section 8 housing
- Excellent oral, writing, and listening skills
- Ability to work independently and initiate corrective actions to help clients seeking permanent housing
- Ability to interact with clients as well as all staff members
- Must complete and pass background record screening process as well as a driving record check
- Must be able to act as a mandatory reporter for child abuse
- Knowledge of and proficiency in Microsoft Word and Excel
- Additional weekend and evening work hours may be required to participate in meetings and events
