Compensation and BenefitsThe estimated salary for this position is $65,000 - $90,000 per year, depending on experience. We also offer a comprehensive benefits package, including health insurance, retirement savings plan, and paid time off.
Job DescriptionAs a Registered Nurse Case Manager at Memorial White Rose Hospice, you will be responsible for developing and implementing individualized care plans for our patients. Your primary focus will be on providing emotional support, pain management, and symptom control to ensure the highest quality of life for our patients and their loved ones.
- You will work closely with an interdisciplinary team to assess patient needs, develop care plans, and provide education to patients and their families.
- Your expertise in end-of-life care will enable you to make informed decisions about patient treatment options and advocate for their best interests.
- You will maintain accurate and timely documentation of patient care, including progress notes, medication lists, and other relevant information.
To succeed in this role, you will need:
- A current RN licensure in the state of Pennsylvania and one year of clinical experience.
- Current CPR certification and a valid driver's license.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Excellent critical thinking and problem-solving skills, with the ability to prioritize tasks and manage competing demands.
