Job Summary: As a Care Manager, you will be responsible for providing high-quality care and services to a consistent group of residents in our assisted living and reminiscence neighborhoods. Your primary focus will be on building meaningful relationships with our residents and their families, ensuring their physical, emotional, and social needs are met.
Responsibilities:
- Develop and implement individualized care plans tailored to each resident's unique needs and preferences.
- Collaborate with the community team to ensure seamless care delivery and address any concerns or issues promptly.
- Conduct regular check-ins with residents and family members to ensure their satisfaction and address any concerns.
- Participate in training and professional development opportunities to enhance your skills and knowledge in gerontology, dementia care, and other relevant areas.
- Maintain accurate records and documentation of resident care, including progress notes, medication administration, and incident reports.
Requirements:
- Dedication to delivering exceptional care and service to seniors.
- Positive attitude, strong communication skills, and ability to work collaboratively as part of a team.
- High School diploma or equivalent required; certifications in gerontology, dementia care, or other relevant areas preferred.
- At least 18 years old and able to lift up to 50 pounds.
