We are seeking a dedicated and empathetic Hospice CHHA - Personal Care Specialist to provide high-quality care to patients in their homes. As a key member of our team, you will work closely with our RN Case Managers to ensure seamless delivery of services.

Key Responsibilities
  1. Deliver personalized care to patients, focusing on their physical, emotional, and social well-being.
  2. Develop and maintain relationships with patients, families, and healthcare professionals to ensure comprehensive care.
  3. Collaborate with our interdisciplinary team to create and implement individualized care plans.
  4. Monitor and report any changes in patient condition, providing timely updates to our RN Case Managers.
  5. Maintain accurate and detailed records of patient interactions and care provided.
