The Position:


The Intake and Case Management Specialist plays a vital role within our organization, functioning as a key member of the Information and Referral (I&R) team.

Your primary responsibility will be to connect clients with suitable community service providers and organizations that facilitate independent living.

Additionally, you will offer case management support for both behavioral health and family clients, encompassing adults and youth.
