Key Responsibilities- Provide Personal Care Services: Assist patients with daily living activities, such as toileting, bathing, dressing, grooming, and oral hygiene.
- Conduct Safety Checks: Ensure the well-being of patients and provide a safe and secure environment.
- Document Clinical Information: Accurately and timely document patient information in their medical record.
- Facilitate Patient Education: Engage patients in educational-rehabilitative groups to promote social skills, coping skills, and independent living skills.
- Provide Transportation: Assist patients with transportation to and from appointments and activities.
- Education: High school diploma or equivalent required.
- Experience: Six months or more experience working with the specific population of the facility preferred.
- Certifications: CPR/First Aid and de-escalation and restraint certification required.
